Hospice Clinical Supervisor
Constellation Health Services is seeking a Hospice Clinical Supervisor to work in our Orange, CT location. Constellation Hospice is a growing program under Constellation Home Care.
The Hospice Clinical Supervisor will be responsible for receiving case referrals, assigning appropriate staff, conferencing with Director of Hospice Services regarding individual patient's eligibility for services, reviews and evaluates clinician documentation and assists clinicians in establishing immediate and long term therapeutic goals in place of care.
The ideal candidate must have a BSN with at least one year of supervisory experience and 3 years of Hospice RN experience. Applicant must have demonstrated leadership, communication, clinical, and technical skills.
